Haven Hospice in Lake City, FL seeks a licensed Practical Nurse (LPN) to join our hospice team for full-time night shifts (7p-7a, 3-12s) with rotating weekends and holidays. This role emphasizes compassionate, patient-centered care and coordination with the hospice team to support patients and families at the end of life.

Formación

• Graduate of an accredited practical nurse or vocational nursing program.
• Three years nursing experience; hospice or medical/surgical experience preferred.
• Currently licensed as an LPN/LVN in the state.
• Comply with accepted professional standards and practice.
• Must be a licensed driver with insured vehicle.
• Possess and maintain CPR certification.
• Understand hospice philosophy and needs of the terminally ill.
• Use knowledge and experience to make decisions and solve problems.
• Demonstrate good verbal and written communication and organization skills.

Responsabilidades

• Give hands-on care to patients nearing end of life, following state nursing rules.
• Follow the care plan after assessing the patient's needs.
• Keep clear and timely records of the care provided.
• Teach and support patients and their families about the illness and care plan.
• Help coordinate care by communicating with the hospice team (aides, chaplains, social workers).
• Manage pain and symptoms, check effectiveness of treatments.
• Take part in training and quality improvement activities to grow professionally and improve care.
