Job summary

Halifax Health Hospice is seeking a Licensed Practical Nurse (LPN) to provide direct patient care to hospice patients across various settings under the supervision of a Registered Nurse. Responsibilities include assessments, documentation, and participation in the interdisciplinary plan of care.

Hours include days, nights, weekends, and holidays as assigned, with on-call rotations. The ideal candidate has an active Florida LPN license, BLS/IV certification, and hospice or medical-surgical

Qualifications

• Valid Florida LPN license is required.
• BLS and IV certification per department requirement.
• Minimum 1 year experience in Medical-Surgical Nursing or equivalent.
• Hospice experience preferred.
• Graduated from an accredited nursing program.

Responsibilities

• Provide direct patient care to hospice patients under RN supervision.
• Perform patient assessments and documentation.
• Participate in the interdisciplinary Plan of Care.
• Contribute to Quality Improvement activities.
• On-call rotations and 24/7 coverage as assigned.
