Job summary

Community Hospice & Palliative Care in Jacksonville, FL, is hiring a Licensed Practical Nurse for the 7 p.m.–7 a.m. shift with alternating weekends.

The role supports hospice care, administers meds, and collaborates with an interdisciplinary team to ensure high-quality patient care, including IV therapy where permitted. Must have IV certification and an active Florida LPN license; a $3,500 sign-on bonus is offered.

Responsibilities

• Assist in implementing and modifying patient care plans with RN and the team.
• Deliver patient-centered care, including administering medications and procedures within LPN scope.
• Monitor and report changes in patient condition to the RN/physician and document it.
• Educate patients and caregivers on appropriate self-care practices.
• Participate in discharge planning and team meetings.
• Maintain patient confidentiality and HIPAA compliance.
• Administer IV therapy as permitted by licensure and training.
