Job summary

Apreva Hospice is seeking a dedicated LPN to provide 1:1 hospice care in San Diego, CA. You will work PRN, delivering hands-on nursing care, following individualized care plans, documenting care, and educating patients and families about conditions and treatments.

Qualifications

• Graduate of an accredited practical nurse or vocational nursing program.
• Three years nursing experience; hospice or medical/surgical background preferred.
• Currently licensed as an LPN/LVN in the State.
• CPR certification maintained where required.

Responsibilities

• Give hands-on care to patients nearing end of life following the care plan.
• Follow the care plan after assessing patient needs.
• Keep clear and timely records of care provided.
• Teach and support patients and families about illness and care plan.
• Coordinate care by communicating with hospice team.
• Manage pain and symptoms and monitor treatment effectiveness.
• Participate in training and quality improvement activities.
