Job summary

BrightSpring Health Services seeks a dedicated RN Case Manager to join our Apreva Hospice team in San Diego, CA. The role involves assessing patient needs, creating individualized hospice care plans, administering care, and coordinating with an interdisciplinary team.

Schedule is Mon-Fri, 8am-5pm, with on-call rotation as required by the branch. Based in a supportive environment, you will provide compassionate, high-quality care to patients and families while pursuing professional growth within

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• 1 year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients
• Create hospice care plans that align with the patients wishes and goals
• Direct nursing care: administering medications, treatments, and interventions
• Provide pain and symptom management
• Educate and support the patient's family and caregivers
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation
• Participate in on-call rotation as required by local branch
