Apreva Hospice in San Diego, CA invites a compassionate Registered Nurse to join our hospice team. You will deliver patient-centered care during a critical time, with the support of a multidisciplinary team and a flexible schedule that supports work-life balance.

We seek candidates with RN licensure, hospice experience, and a commitment to clinical excellence. You’ll provide 1:1 care, develop care plans, manage symptoms, and educate families in a dignified, compassionate environment.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year nursing experience required, 3+ years preferred
• Experience in hospice or a similar setting preferred
• Knowledge of the hospice philosophy of care
• Commitment to clinical and documentation excellence
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
• Possess and maintain current CPR certification in states where required.

Responsabilités

• Assess/ monitor physical, emotional, and psychological needs of patients
• Create hospice care plans that align with the patients wishes and goals
• Direct nursing care: administering medications, treatments, and interventions
• Provide pain and symptom management
• Educate and support the patient’s family and caregivers
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation
• Participate in on-call rotation as required by local branch
