Job summary

Apreva Hospice is seeking a dedicated Hospice LPN to join our San Diego team. You will provide compassionate, patient-centered end-of-life care in a supportive environment and collaborate with a caring multidisciplinary team.

This PRN position offers flexible scheduling, opportunities for professional growth, and a stable work setting with a strong focus on dignity and quality of life for patients and families in San Diego County.

Qualifications

• Graduate of an accredited practical nurse or vocational nursing program.
• Three years nursing experience. Community health/hospice or medical/surgical experience is preferred.
• Currently licensed as an LPN/LVN in the State.
• Must be a licensed driver with an automobile that is insured and in good working order.
• Possess and maintain CPR certification in states where required.
• Understand philosophy of hospice care and needs of the terminally ill.
• Strong verbal and written communication and organization skills.

Responsibilities

• Give hands-on care to patients nearing the end of life, following state nursing rules.
• Follow the care plan after assessing the patient's needs.
• Keep clear and timely records of the care provided.
• Teach and support patients and their families about the illness and care plan.
• Help coordinate care by communicating with the hospice team (aides, chaplains, social workers).
• Manage pain and symptoms, checking how well treatments are working.

As a Hospice LPN You will:

Qualifications

• Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
• Possess and maintain current CPR certification in states where required.
• Understand philosophy of hospice concept. Also understand needs of the terminally ill.
• Use own knowledge and experience base and other resources as necessary to make logical decisions and solve problems.
• Demonstrate good verbal and written communication, and organization skills.
