ViaQuest Hospice seeks a licensed practical nurse (LPN) to support RNs and physicians in providing compassionate hospice care across multiple sites in Somerset, Greensburg, Delmont, and North Huntingdon, PA.

You will administer medications, maintain accurate records, and coordinate care with the RN to meet patient and family needs. A valid LPN license and willingness to travel throughout the service area are required.

Qualifications

• Graduate of an accredited nursing program.
• LPN license.
• Nursing or patient care experience preferably within Hospice.
• Valid driver's license, car insurance and reliable transportation.
• Willingness to travel throughout assigned service area.

Responsabilités

• Assisting RNs or Physicians in performing specialized procedures, duties and plan of care items.
• Administering and instructing administration of medication.
• Maintaining current patient records and progress notes.
• Maintaining regular communication with RN concerning patient and family care needs.
