Job summary

ViaQuest Hospice in West Lafayette, IN is seeking a registered nurse to provide comprehensive hospice care. You will support patients and families physically and emotionally, manage caseloads, and coordinate with physicians and the care team.

The role requires an RN license, an Associate degree in Nursing, and hospice experience is preferred. Travel within the service area, a valid driver’s license, and reliable transportation are expected.

Qualifications

• RN license required.
• Associates degree or equivalent in Nursing from an accredited program.
• Hospice nursing experience preferred.
• Valid driver’s license, car insurance and reliable transportation.

Responsibilities

• Provide physical and emotional support to patients and families.
• Manage a patient/family caseload including assessments, planning, implementation and evaluation of the nursing process.
• Administer and instruct medication administration.
• Coordinate care by communicating with physicians, hospice team members and other agencies.
• Maintain current patient records and progress notes.
• Supervise LPNs and Hospice Aides and document delivery of care.
