Bristol Hospice in Honolulu is seeking a compassionate RN Case Manager to provide exceptional hospice care. You will develop individualized care plans and ensure dignified, one-on-one support for patients and families in need.

Qualifications

• Valid RN license in good standing in Hawaii.
• Current CPR certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Develop and implement individualized care plans.
• Deliver one-on-one support to patients and families.
• Conduct assessments of physical, emotional, and spiritual needs.
• Monitor and document vital signs accurately.
• Administer medications and treatments with care.
• Educate patients on care techniques and symptom management.
• Track changes in patient condition and communicate effectively.
• Maintain accurate clinical documentation.
• Collaborate with interdisciplinary team members.
• Perform additional duties as assigned.

Qualifications and Duties

• Develop and implement individualized care plans based on current and prior medical history
• Deliver attentive, one-on-one support to patients and their families, ensuring comfort, dignity, and understanding
• Conduct thorough assessments of physical, emotional, and spiritual needs throughout the care journey
• Accurately monitor and document vital signs including temperature, pulse, respiration, and blood pressure
• Administer physician-ordered medications and treatments with skill and empathy
• Educate patients and their families on care techniques and symptom management
• Track and communicate changes in patient condition, ensuring timely and effective responses
• Maintain detailed and accurate clinical documentation reflecting patient progress and quality of care
• Collaborate with interdisciplinary team members to ensure comprehensive care delivery
• Perform additional duties as assigned in support of patient care and team success

• A valid Registered Nurse (RN) license in good standing in the state of Hawaii
• Current CPR certification
• A valid driver's license and reliable transportation
