• Must be a graduate from an accredited practical nurse or vocational nursing program.
• Must hold a valid/active license as a Practical Nurse (LPN) or LVN in Ohio.
• Prior hospice experience is highly preferred.
• Prior experience in community/home health and/or medical/surgical, preferred.
• 3 years of nursing experience, preferred.
• Comply with accepted professional standards and practices.
• Demonstrates good verbal and written communication, and organizational skills.
• Must be able to lift, position or transfer patients and lift supplies and equipment.

Responsibilities

• Provide direct patient care under the supervision of a registered nurse.
• Follow the plan of care, provide treatments, and work collaboratively with the team to meet patient outcomes.
• Implement the plan of care initiated by the registered nurse.
• Provide accurate and timely documentation consistent with the plan of care.
• Educate patient and family/caregiver on self-care and plan of care.
• Coordinate home health and hospice services and report needs to the RN or Clinic Manager.
• Use and prepare equipment and supplies adhering to aseptic techniques.
• Participate in personal and professional growth and development.
• Perform other duties assigned by the RN, including caring for home care patients as census allows.
• Participate in weekend and holiday on-call rotation requirements.
