Job summary

SolutionHealth is seeking a Licensed Nursing Assistant (LNA) to provide delegated nursing and homemaking support to patients in their homes across New Hampshire. You will perform basic patient care, assist with treatments, and ensure documentation is accurate and timely.

A $5,000 bonus is eligible and a NH nursing assistant license is required. The role involves travel within NH, weekend/evening/in‑call coverage as needed, and collaboration with a multidisciplinary team to support dignity and

Qualifications

• High school diploma or equivalent required or preferred.
• Experience in home health or caregiving is a plus.

Responsibilities

• Adheres to the patient/home care plan under the primary nurse/therapist guidance.
• Carries out patient care/treatments with competency.
• Reinforces teaching to caregivers and patients on personal care techniques.
• Integrates problem-solving and quality improvement in patient care and processes.
• Completes documentation in clinical records, time sheets and quality logs.
• Attends at least 12 hours of in-services per year.
• Acts as a resource to others in provision of client/patient care.
