Job summary

Care360 Hospice is seeking a Licensed Practical Nurse for after-hours, part-time nights in Hilliard, Ohio. The role includes on-call visits and skilled nursing for end-of-life patients, with Saturdays 8 PM–8 AM. The LPN will assist RNs, support the care plan, and travel as needed to patient homes or facilities.

Requires an LPN degree, at least 1 year of clinical experience, and a valid driver’s license with auto insurance. PTO and growth opportunities are offered.

Qualifications

• Licensed Practical Nursing degree is required.
• Minimum of 1 year of clinical experience.
• Hospice experience preferred.
• Valid driver's license and auto insurance coverage.
• Reliable transportation and willingness to travel.

Responsibilities

• Provide skilled nursing care to EOL patients in facility or home visits.
• Provide on-call services after hours and complete skilled visits.
• Assist RNs in carrying out the plan of care.
• Assist RN with procedures under physician orders.
• Collaborate to keep the patient care plan current and address issues.
• Educate patients and families about end-of-life care.
• Engage in ongoing professional development and nursing meetings.

Hospice Licensed Practical Nurse After-hours

Salary: $31.25-$34.13 BOE

Schedule: Part-time | Saturdays | 8 PM-8 AM

Location: Hilliard, Ohio, and surrounding area

Requirements of the Hospice LPN role:

• Licensed Practical Nursing Degree
• A minimum of 1 year of Clinical experience
• Hospice experience preferred
• Valid driver's license and Auto insurance Coverage
• Reliable transportation: Willing to travel

About the Hospice LPN ro le:

• Provide skilled nursing care to patients in EOL, conducting visits in a facility and/or home patients.
• Provide on-call services after hours, and complete skilled visits.
• Assist Registered nurses in carrying out the plan of care.
• Assist RN in performing specialized procedures( as defined in the state nurse practice act) as directed by the RN under physician's orders.
• Actively participate in the IDGH in collaborating to ensure the patient plan of care is kept current and identified issues are addressed.
• Assist in the reinforcement of patient and family education regarding end-of-life care.
• Assume responsibility for personal and vocational growth and development through active participation in nursing organizations and participating in in-service programs and other on-the-job learning.
• Keep the RNCM informed of changes noted during the process.
• Participate in self-development by practicing further education, workshops, and conferences, active in professional and related organizational meetings, and individual research reading.
