Job summary

Care 360 Hospice is seeking a dedicated LPN for night shifts serving Kettering, Ohio and surrounding areas. The role requires a valid LPN license, at least 1 year in skilled nursing, and willingness to travel with reliable transportation. Hours are 8 PM–8 AM, Fri/Sat/Sun, full-time.

Qualifications

• Must be a Licensed Practical Nurse in state of practice.

Responsibilities

• Provide skilled nursing care to EOL patients in facility or home settings.
• Provide after-hours on-call services and complete scheduled visits.
• Respond to patient and family calls regarding symptoms, changes, or emergencies.
• Assess patients with critical symptoms and manage pain, respiratory distress, or emotional crises.
• Assist RNCM with plan of care and carry out specialized procedures as directed by the RNCM.
• Support education for patients and families on end-of-life care.
• Participate in professional development and nursing organization activities.
• Assist with emergent visits including continuous care and companion care as needed.
• All other duties as assigned.

LPN Salary: $31.25-$34.13/hr BOE

Schedule: Full-time | Fri/Sat/Sun | 8 PM-8 AM

• Must be a Licensed Practical Nurse in state of practice
• A minimum of 1 year of experience in skilled nursing
• Hospice Experience Preferred
• Valid driver's License and Auto insurance coverage
• Reliable transportation: Willing to travel

About the Hospice Nurse role:

• Provide skilled nursing care to patients in EOL, conducting visits in a facility and/or home patients.
• Provide on-call services after hours, and complete scheduled skilled visits.
• Responding to phone calls from patients and their families regarding concerns about symptoms, changes in conditions, or emergencies.
• Conducting home/facility visits to assess patients with critical symptoms such as pain management, respiratory distress, or emotional crisis.
• Assist RNCM in carrying out the plan of care.
• Assist the Nursing team in performing specialized procedures( as defined in the state nurse practice act) as directed by the RNCM under the physician's orders.
• Actively participate in the IDGH in collaborating to ensure the patient plan of care is kept current and identified issues are addressed.
• Assist in the reinforcement of patient and family education regarding end-of-life care.
• Assume responsibility for personal and vocational growth and development through active participation in nursing organizations and participating in in-service programs and other on-the-job learning.
• Keep the RNCM informed of changes noted during the process.
• Participate in self-development by practicing further education, workshops, and conferences, active in professional and related organizational meetings, and individual research and reading.
• Assist with emergent visits including continuous care (CC) and companion care as needed.
• All other duties as assigned
