Job summary

Gilchrist Hospice Care seeks a Hospice Liaison Nurse to coordinate admissions from hospital referrals and guide patients and families through transitions with dignity and comfort. This full-time role focuses on University of Maryland St.

Joseph Medical Center in Towson, MD, and requires building external relationships with hospitals and care teams. The role demands a MD RN license, 3+ years of clinical experience in hospice/acute care settings, and strong communication, assessment, and EMR

Qualifications

• RN with MD license; associate degree or nursing diploma.
• Minimum 3 years of clinical experience; hospice/home health/ICU/ER/oncology or similar.
• Ability to transition patients to hospice and collaborate with physicians, social workers and nurses.
• Proficient with EMR (EPIC) and comfortable with computers.

Responsibilities

• Actively develops and fosters area hospital referral source acceptance and allegiance to Gilchrist.
• Represents Gilchrist professionally and responds promptly to referral source questions or requests for presentations.
• Maintains knowledge of discharge planning processes and builds relationships with referral sources.
• Professionally and compassionately interacts with patients, families and hospital referral sources to explain end-of-life care services.
• Delivers in-services, lunch & learns and other presentations educating the medical community on GHC and hospice care.
