Job summary

Gilchrist Hospice Care in Towson, MD seeks a Hospital Liaison Nurse to generate and coordinate admissions from hospital referrals, guiding patients and families through transitions with compassion.

You will build external hospital relationships, provide clinical assessments, and conduct in-services for hospital staff, using EPIC EMR to document care and ensure smooth transitions to hospice services. This role is full-time with Monday–Friday schedules and no on-call or weekend responsibilities.

Qualifications

• MD RN license is required.
• Minimum of three years of clinical experience in hospice, home health, critical care or acute care.
• Ability to manage multiple tasks in a fast-paced environment.
• Strong communication and presentation skills for in-services and education.

Responsibilities

• Generate and facilitate hospital admissions from referrals.
• Conduct clinical assessments for prospective patients.
• Represent Gilchrist with hospitals and build external relationships.
• Deliver in-services and lunch-and-learns to medical staff.
• Document care using EMR (EPIC) consistently.
