Job summary

Bristol Hospice in Peninsula Area is seeking a Hospice Liaison to build relationships with referral sources across clinics, hospitals, and community settings. You will travel in your own vehicle to connect patients and families with end-of-life care.

Whether you come from healthcare or sales, you will thrive on trust-building and delivering results. Flexible scheduling with some evenings and weekends; mileage reimbursement provided.

Qualifications

• Minimum of 1 year in sales, marketing, professional relations or education training.
• Experience in hospice sales, home health sales, medical sales or clinical liaison is preferred.

Responsibilities

• Build relationships with assisted living facilities, hospitals, physician offices and referral sources.
• Be out in the community daily, representing Bristol Hospice.
• Make outreach calls and deliver impactful presentations to referral sources.
• Collaborate with clinical and operations teams to ensure seamless care.
• Inform patients and families about hospice care options.
• Meet with community partners about hospice services.
• Participate in regular team calls and grow Bristol Hospice's presence.
• Develop relationships of trust and integrity within the community.
