Job summary

Bristol Hospice is seeking a Care Consultant (Hospice Liaison) to build relationships with referral sources and connect patients and families with hospice care across our service area. You will travel in your own vehicle, presenting to hospitals, assisted living facilities, physician offices, and community partners.

The ideal candidate has at least one year of sales, marketing, or professional relations experience, with a passion for compassionate care and strong communication skills.

Qualifications

• Minimum of 1 year of sales, marketing, professional relations and/or education training required.
• Experience in hospice sales, home health sales, medical device sales, clinical liaison, RN liaison, clinical coordination or medical sales is a plus.

Responsibilities

• Build relationships with referral sources such as assisted living facilities, hospitals, physician offices and personal care homes.
• Be the face of Bristol Hospice in the community; conduct outreach and deliver impactful presentations.
• Collaborate with clinical and operational teams to ensure seamless care and promote clinical excellence.
• Ensure patients and families receive clear information about hospice care.
• Meet with community partners to discuss hospice services for their patients.
• Participate in regular collaborative team calls to align outreach efforts.
• Develop and maintain relationships of trust with community partners to drive growth through advocacy.
