Job summary

Ohio's Hospice, Inc. is seeking a Hospice Patient Care Specialist on the IPU for a full-time night shift in Franklin, OH. You will work with an interdisciplinary team to deliver compassionate care that brings comfort, dignity, and peace to patients and their families during some of life's most important moments.

This FT night position runs 7pm–7:30am with every other weekend/holiday rotation. Mileage reimbursement, company-provided technology, and ongoing education are offered to support

Qualifications

• High School Diploma or GED is required.
• Proficient computer skills with ability to document patient care in EMRs.
• Certified Nurse Aide (preferred, but on-the-job training available).
• CHPNA certification preferred; Ohio's Hospice supports certification growth.

Responsibilities

• Deliver compassionate, patient- and family-centered care as part of an interdisciplinary team.
• Accurately document patient care and services in electronic medical records.
• Support families and patients during critical moments with dignity and empathy.
• Contribute to a supportive, high-quality care culture on IPU units.

Hospice PCS - Making every moment matter

As a Hospice PCS, you will play a vital role in enhancing quality of life for patients and supporting families during some of their most important moments. Working alongside a dedicated interdisciplinary team, you'll deliver compassionate care that brings comfort, dignity, and peace.

This is a FT night shift position, 7pm-7:30am with every other weekend/holiday rotation. This position is located at our Middletown Hospice House IPU and float to our Dayton Hospice House IPU when needed as well.

Qualifications of this role:

• High School Diploma or GED
• Certified Nurse Aide (preferred, but will train internally)
• Certified Hospice and Palliative Nurse Aide (CHPNA) certification preferred. Ohio's Hospice supports professional growth and offers opportunities for eligible employees to obtain certification.
• Proficient computer skills with the ability to accurately document patient care and services in electronic medical records.
• Valid Ohio driver's license with a safe driving record and proof of auto insurance.
• Ability to pass a criminal background check and 10 panel drug screening.
