Job summary

Suncrest Hospice is seeking an Intake Coordinator in Walnut Creek, CA to manage phone inquiries, process referrals, and assist with chart reviews. You will support scheduling goals of care and admissions visits, coordinate with the sales team for referral information, and help maintain accuracy in the electronic medical records system.

The role requires at least two years of hospice/healthcare experience or a two-year college degree, strong customer service skills, and proficiency with

Qualifications

• Minimum of two years’ experience preferably in hospice or health care setting, or graduation from a one- or two-year college preferred.
• Experience working with insurance
• Proficient with Microsoft applications including Word and Excel
• Strong time management and organization skills
• Strong ability to multi task and prioritize
• Excellent telephone and customer service skills
• Proficient with medical terminology

Responsibilities

• Answers telephone inquiries and channels them appropriately
• Assists with tracking and processing referrals
• Assist with audits and chart reviews as assigned
• Uploads documents to electronic medical records system
• Provides feedback on process and flow improvement
• Assists with scheduling goals of care and admission visits
• Work with sales team to receive incoming referral information
• Assists on weekends/holidays as needed
• Other duties as assigned by Director of Intake
