Compassus is seeking an Intake Assessment LVN with home health experience to manage referrals, coordinate admissions, and ensure compliance with Medicare and state regulations. The role requires strong communication, organization, and clinical coordination with the Nursing leadership team.

The LVN will handle referrals, coordinate with QA and eligibility for initial visit authorization, and help maintain intake training materials.

Qualifications

• Coursework/Training to attain LVN licensure in California.
• 1 year Acute care experience.
• 1 year Home health experience.

Responsibilities

• Prioritize daily referrals to guarantee timely patient admission.
• Answer telephone calls promptly and courteously.
• Check and follow up on ECIN and iNEXX referrals daily.
• Scan referral information into All Docs.
• Transcribe medical and insurance information from faxed documents and calls.
• Verify and obtain orders from physicians.
