Job summary

Crossroads Hospice & Palliative Care in Akron is seeking an Intake Coordinator to manage referrals, schedule patient assessments, and coordinate with care teams. The role emphasizes compassion, accuracy, and timely processing in a high-volume environment.

Candidates should have at least 3 years hospice experience (intake preferred) and proficiency with Teams, SharePoint, and electronic health records. This full-time on-site position offers competitive pay and comprehensive benefits.

Qualifications

• At least 3 years of hospice experience (intake preferred, billing acceptable) in a high-volume referral setting
• Receive and promptly process hospice referrals and inquiries, scheduling a hospice representative with the patient and family to assess the need for services
• Assign patients to the appropriate team after admission, in coordination with team leaders
• Ready to apply hospice experience to the workflows of a centralized system; central office experience is a plus, including handing off and receiving incomplete work items
• Strong working knowledge of Teams, SharePoint, and electronic clinical records
• Able to work across multiple systems and juggle competing priorities with strong attention to detail
• Compassionate, professional phone presence
• Reliable transportation, a valid driver's license, and the ability to safely operate a vehicle

Intake Coordinator Schedule:

Tuesday to Saturday

• Tuesday to Friday: 10:00 AM - 6:30 PM
• Saturday: 11:00 AM - 7:00 PM
