Job summary

MJHS in New York, NY seeks an experienced Hospice Inpatient Clinical Coordinator to lead nursing coordination for General Inpatient (GIP) care and collaborate with hospital partners.

You will assess referrals, develop care plans with an interdisciplinary team, educate referrers and families, and ensure documentation aligns with regulations.

Requirements include NYS RN license, BSN preferred, CPR-BLS, and ability to work flexible hours, including weekends.

Qualifications

• Graduate from an accredited School of Nursing; BSN preferred.
• Two years’ experience in Medical-Surgical or Community Health; hospice/oncology preferred.
• New York State RN license and CPR-BLS.
• Valid NYS Driver's license preferred.
• Familiarity with Microsoft Office applications.
• Ability to work flexible hours, including weekends.

Responsibilities

• Develops strong business continuity relationships to support the agency’s business development goals.
• Responds to all information requests and referrals from hospital personnel and assists with hospice enrollment.
• Gathers clinical information used in eligibility determinations per Hospice CO P, LCDs, and regulations.
• Coordinates symptom management for patients awaiting discharge to home hospice with referring physicians and hospital staff.
• Serves as the RN member of the Inpatient Hospice IDT, coordinating GIP care.
• Develops the hospice nursing plan of care and makes recommendations based on patient/family needs.
• Participates in IDT meetings weekly and GIP huddles daily to update patient status.
• Coordinates transition plans for patients no longer qualifying for inpatient care.
• Oversees clinical care provided by hospital staff and educates on hospice care to direct care staff.
• Maintains accurate and current documentation of hospice nursing assessments and interventions.
