Job summary

MJHS is seeking an experienced Hospice Inpatient Clinical Coordinator to strengthen hospital partnerships, assess referrals, and provide GIP level care for hospice in-patients with expert clinical skills and compassion.

The role includes conducting comprehensive assessments, coordinating with inpatient providers, educating families, and ensuring compliant documentation within the interdisciplinary team framework.

Qualifications

• Graduate from an accredited School of Nursing; BSN preferred.
• Minimum two years’ experience in Medical-Surgical or Community Health; hospice or oncology nursing preferred.
• New York State RN licensure, CPR-BLS.
• Valid NYS Driver's license preferred.
• Familiarity with Microsoft Office applications.

Responsibilities

• Develops strong business continuity relationships to support the agency’s business development goals and responds to requests for information from hospital personnel.
• Assess patients referred for hospice enrollment and provide General Inpatient (GIP) level of care.
• Coordinate with hospital case management and referring physicians to manage transition plans.
• Develop hospice nursing plans of care and coordinate with the interdisciplinary team (IDT).
• Educate referrers, patients, and families about hospice services and eligibility.
• Maintain accurate documentation reflecting hospice nursing assessments and interventions.
