Job summary

Trustbridge Hospice, a member of Empath Health, is seeking a caring and motivated Hospice Registered Nurse (RN) Case Manager to join our dedicated hospice team in the Palm Beach County area. The schedule is Monday–Friday 8:30am–5:00pm with a weekend requirement.

You will coordinate patient-centered care under the direction of the primary physician, collaborate with the interdisciplinary team, and educate families to enhance comfort, dignity, and quality of life at end of life.

Qualifications

• Florida RN license with active Basic Life Support (BLS) certification.
• Experience in hospice, public health, home health, oncology, or geriatrics preferred.
• Strong communication and teaching skills with families.
• Proficiency with EMR systems is a plus.
• Reliable transportation and valid Florida driver's license.

Responsibilities

• Advocate for patients and families to ensure needs are recognized.
• Coordinate comprehensive care with the primary physician and IDG.
• Develop and implement individualized care plans as part of the IDG.
• Monitor patient conditions, provide interventions, and educate to enhance comfort and quality of life.
• Address concerns promptly to maintain satisfaction and trust.

Trustbridge Hospice, a member of Empath Health, is seeking a caring and motivated Hospice Registered Nurse (RN) Case Manager to join our dedicated hospice team in the Palm Beach County area. In this vital role, you will provide exceptional, patient-centered care while guiding families through one of life’s most meaningful journeys on the following schedule: Monday - Friday 8:30am - 5:00pm with a weekend requirement.

What You'll Do

• Serve as a key advocate for patients and families, ensuring their needs are recognized and addressed
• Coordinate comprehensive care under the direction of the primary physician and in collaboration with the interdisciplinary team
• Develop and implement individualized plans of care as part of the IDG (Interdisciplinary Group)
• Use your clinical expertise to monitor patient conditions, provide interventions, and deliver education that enhances comfort and quality of life
• Address concerns promptly and thoughtfully to ensure a high level of satisfaction and trust

What You'll Need

• Registered Nurse currently licensed in the State of Florida.
• Current Basic Life Support Certification.
• One year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing preferred.
• Understanding of and ability to apply characteristics of Hospice Nursing: case management, crisis intervention, communication skills, teaching skills, thanatology skills and needs
• Basic computer skills and EMR software experience a plus.
• Adequate and reliable means of transportation with adequate insurance and valid Florida driver's license.
