Beth Israel Lahey Health is seeking an Overnight Hospice Registered Nurse in Reading, MA, to provide professional nursing care within the hospice team. The RN will assess, plan, and evaluate nursing services under supervision of the Hospice House RN Manager, ensuring care aligns with the plan of care for assigned patients.

You’ll perform visits, apply palliative care principles, monitor patient status, educate families, and participate in IDT meetings while upholding HIPAA and confidentiality

Formación

• Active Massachusetts RN license.
• BLS certification required.
• 2+ years of RN experience in acute care, rehab or home care.
• Hospice experience preferred.

Responsabilidades

• Perform nursing visits to assigned patients under Hospice care.
• Provide hospice nursing care with symptom management and palliative care principles.
• Observe, record, and report significant changes to MD and Hospice RN Manager.
• Educate patients and families as required by the plan of care.
• Communicate effectively with patients, caregivers, and the care team.
• Attend regular IDT meetings.
• Maintain confidentiality per HIPAA guidelines.

Under the direction and supervision of the Hospice House RN Manager, the Hospice RN is accountable for the quality of nursing care he or she provides and assumes responsibility for the provision of nursing care in accordance with the established plan of care for assigned patients.More Specifically:Perform nursing visits to patients as assigned by the Hospice RN Case Manager/Hospice Clinical Manager.Provide hospice nursing care to patients and families utilizing current principles of palliative care and symptom management to produce hospice outcomes as directed.Observe, record, and report significant changes in the patient’s healthcare status to the MD and Hospice RN Manager.Participate in the health teaching required by the patient and family.Respect and support patient and family’s cultural beliefs and values, individual needs, health goals and treatment preferences.Effectively communicate with patients, caregivers, and members of the interdisciplinary healthcare team incorporating interpersonal and therapeutic communication skills.Attend regular IDT meetings.Practice confidentiality principles set by the agency and federal HIPAA guidelines.What You’ll Need:An active valid Massachusetts RN license;An active American Health Association BLS certification;2+ years of experience as an RN in an acute care, rehab or home care setting; 1+ year of hospice experience preferred.What You’ll Get:A highly competitive pay rate & benefits package, including generous PTO, 403(b), and tuition reimbursementA reasonable geographic territory with strong clinical support resourcesA highly inclusive, diverse team that values the input of all staff to provide excellent patient carePay Range:$78,604.50 USD – $112,500.00 USDThe pay range listed for this position is the annual base salary range the organization reasonably and in good faith expects to pay for this position at this time.
