Job summary

Corewell Health in Grand Rapids seeks a compassionate RN to assess, plan, implement, coordinate, monitor and evaluate end-of-life care strategies for patients and families, ensuring a pain-free, peaceful death. You will lead the care plan with the interdisciplinary team and supervise LPNs/CENAs.

The role emphasizes patient-centered care, advocacy for symptom management, and education to families, while aligning with our mission and standards of care.

Qualifications

• Required Associate's Degree Nursing
• Preferred Bachelor's Degree Nursing
• 1 year of relevant experience, preferably in hospice or related field. Required
• Experience with dying patients and their families. Preferred
• LIC-Registered Nurse - STATE_MI State of Michigan required
• CPR - AHA American Heart Association 90 Days required

Responsibilities

• Exhibit "Patients and Families First" attitude. Responds with compassion, flexibility and urgency to each patient need.
• Develop a comprehensive plan of care that addresses physical, emotional, cultural and spiritual needs of the patient/family with input from the patient/family and the interdisciplinary team.
• Advocate for pain/symptom management.
• Educate and provide the support to patient/family regarding care issues. Committed to the continuous improvement of the patient experience which includes patient safety, quality of care and excellent service. Adheres to the organization's Mission, Service Standards and Code of Conduct.
• Direct and supervise LPN and CENA.
• Administer and document medications/treatments as ordered by the physician according to policies in an accurate and timely manner.
• Identify and respond appropriately to emergency situations.
• Attend shift reporting and IDT meetings as necessary to facilitate communication of decisions related to the patients care.
