Job summary

Corewell Health is seeking a Registered Nurse for continuing care in Grand Rapids to assess, plan, implement, and evaluate end-of-life care for patients and families. The role emphasizes a holistic, relationship-centered approach and coordination with the interdisciplinary team to provide a peaceful and dignified death.

The ideal candidate holds an ADN (BSN preferred) with 1 year of hospice experience and an MI RN license.

Qualifications

  • Must hold an Associate's Degree in Nursing (ADN) or equivalent.
  • Bachelor's Degree in Nursing preferred for advancement.
  • 1 year of relevant hospice or related field experience required.
  • Experience with dying patients and their families required.

Responsibilities

  • Assess and care for patients in end-of-life care using nursing process.
  • Develop and adjust comprehensive plan of care with input from patient/family and team.
  • Advocate for pain and symptom management.
  • Direct and supervise LPN and CENA; administer medications as ordered.

Job description

Job Summary

The RN is responsible for assessing, planning, implementing, coordinating, monitoring and evaluating the strategies and services required to meet the patients/family's end-of-life care needs. An outcome based plan of care is created and continually adjusted through utilization of the nursing process, relationship centered care principles, and holistic philosophy, assuring that the changing needs of the patient/family are met while facilitating a pain‑free, peaceful, sacred death.

Essential Functions
  • Exhibit “Patients and Families First” attitude.
  • Responds with compassion, flexibility and urgency to each patient need.
  • Provides the care that patients want, when they want it.
  • Develop a comprehensive plan of care that addresses physical, emotional, cultural and spiritual needs of the patient/family with input from the patient/family and the interdisciplinary team.
  • Evaluate this plan and makes interventions as necessary.
  • Advocate for pain/symptom management.
  • Educate and provide the support to patient/family regarding care issues.
  • Committed to the continuous improvement of the patient experience which includes patient safety, quality of care and excellent service.
  • Adheres to the organization’s Mission, Service Standards and Code of Conduct.
  • Direct and supervise LPN and CENA.
  • Administer and document medications/treatments as ordered by the physician according to policies in an accurate and timely manner.
  • Identify and respond appropriately to emergency situations.
  • Attend shift reporting and IDT meetings as necessary to facilitate communication of decisions related to the patients care.
Qualifications
  • Associate's Degree Nursing (Required)
  • Bachelor's Degree Nursing (Preferred)
  • 1 year of relevant experience experience, preferably in hospice or related field. (Required)
  • Experience with dying patients and their families. (Required)
  • Preferred LIC-Registered Nurse (RN) - STATE_MI State of Michigan required
  • CRT-CPR - AHA American Heart Association 90 Days required
