Qualifications

• Must have active patient contact within the past year.
• Completion of a nursing refresher course within the past year if applicable.
• Current South Carolina Driver’s License and Automobile Insurance as required by law.

Responsibilities

• Maintain a professional image and exhibit excellent customer relations.
• Assess the needs of patients and develop a plan of care.
• Serve as a case manager and interdisciplinary leader for assigned patients.
• Recognize signs and symptoms of actively dying patients.
• Participate in on-call rotation to maintain 24/7 RN availability.
• Complete documentation and sync electronic devices according to policy.
