Job summary

Agrace is seeking an experienced CNA for Homecare in Madison, Wisconsin. The role involves assisting patients with daily living activities, light homemaker tasks, and documenting care in the electronic health record.

You will follow the plan of care and communicate changes to the Interdisciplinary Team. The position offers a daytime schedule, with weekend and holiday rotations, competitive pay, and a $3,000 sign-on bonus.

Qualifications

• High school diploma or GED or equivalent work experience.
• Graduation from an accredited CNA program.
• Current, unrestricted Nursing Assistant Certification in state of employment.
• One-year caregiver or hospice/home health experience preferred.
• Valid driver's license and reliable vehicle.

Responsibilities

• Assist with activities of daily living and ambulation.
• Provide light homemaker tasks and prepare meals.
• Document care and changes in electronic health records.
• Communicate with the care team about patient conditions and needs.
• Participate in meetings and precept new staff.
