Job summary

St. Croix Hospice is seeking a Registered Nurse Case Manager to provide in-home, holistic hospice care.

You will collaborate with the care team, assess patient needs, and create personalized plans of care to support patients and families during end-of-life journeys. Responsibilities include conducting comprehensive assessments, educating caregivers, managing symptoms, and participating in on-call rotations while maintaining compassion and clinical excellence.

Qualifications

• Current RN license in Iowa.
• 1+ years of acute care experience; hospice/home health preferred.
• Must have reliable transportation and valid driver’s license.
• Ability to perform comprehensive assessments and develop care plans.
• On-call duties may be required.

Responsibilities

• Develop individualized care plans with patients and families and update as needs evolve.
• Lead meaningful end-of-life conversations with patients and families.
• Ensure symptom management with empathy and clinical excellence.
• Administer medications and treatments as prescribed.
• Educate patients and caregivers on care techniques and medications.
• Coordinate care with the interdisciplinary team and ensure smooth transitions during on-call periods.
