Job summary

Central Coast VNA & Hospice is seeking a Licensed Vocational Nurse (LVN) to work under the direction of the patient’s physician and RN Case Manager to deliver individualized care and support the treatment plan. The LVN will promote comfort, independence, and quality of life for patients in a home health setting on a full-time basis.

Responsibilities include assessing patient status, reporting changes, administering medications, and coordinating supplies, while adhering to laws and agency

Qualifications

• Current California Vocational Nurse license.
• At least one year of LVN experience; experience in a home health, hospice, or applicable program preferred.
• Current CPR certification.

Job Summary: The Licensed Vocational Nurse (LVN) will work under the direction of the patient’s physician and RN Case Manager to deliver individualized care, support the plan of treatment, and promote comfort, independence, and quality of life.

Responsibilities include assessing and monitoring the patient’s status, promptly reporting changes in condition to the RN Case Manager or Clinical Supervisor. Administers prescribed medications and treatments in accordance with applicable laws and agency standards. Monitors and maintains appropriate medical supplies and coordinates replenishments as needed.

Status: Full-Time (exempt)

Education/Experience/Skills/Qualifications:

• Current California Vocational Nurse license
• At least one year of LVN experience; experience in a home health, hospice, or applicable program preferred
• Current CPR certification

Special Requirements:

• Current driver's license
• Reliable vehicle

The salary range provided reflects the lowest to highest amount we reasonably anticipate paying for this position at the time of posting.
