Job summary

VNA Health is seeking a Licensed Vocational Nurse (LVN) to provide direct patient care under the supervision of an RN in our hospice program. You will follow the plan of care, administer treatments, and collaborate with the care team to achieve positive patient outcomes.

The ideal candidate holds a California LVN license, CPR certification, and at least 1 year of nursing experience, with Spanish language skills preferred.

Qualifications

• California LVN license active and current.
• CPR certification current.
• Minimum one year nursing experience; three years preferred.
• Spanish language skills preferred.
• Valid driver’s license with insured automobile.

Responsibilities

• Provides direct patient care under the supervision of a Registered Nurse.
• Implements care plan and treatments per plan of care.
• Documents patient services accurately and timely to reflect the plan of care.
• Educates patients and families on diagnosis and self-care.
• Collaborates with hospice team to coordinate care.
