Job summary

Columbus Hospice of Georgia and Alabama is seeking an RN for Home Care to provide individualized nursing care in accordance with hospice philosophy. You will educate patients and families, perform nursing assessments, and collaborate with the Interdisciplinary Team to manage symptoms and plan care.

The role includes taking after-hours on-call shifts, dealing with grief and end-of-life emotions, and maintaining a flexible schedule while ensuring high-quality patient care.

Qualifications

• Must hold valid Alabama and Georgia Nursing License or multi-state compact RN license.
• Valid driver’s license and good driving record, and proof of insurance
• Current BLS CPR certification
• Influenza vaccine during influenza season (Oct. – March)

Responsibilities

• Provide individualized nursing care in accordance with Hospice philosophies and standards, educate patients and families, conduct nursing assessments, manage symptoms, and collaborate with the Interdisciplinary Team.
• Take call on rotating basis after hours and holidays for patient care issues and make visits as needed.
• Maintain flexibility to evaluate situations, prioritize and coordinate patient care needs daily.
• Keep the attending physician informed of patient status and request care changes as needed.
