Job summary

GBMC HealthCare is seeking a Hospice Home Care Nurse to identify patient and family needs and provide nursing and supportive care for terminally ill patients and families. The role requires an ADN/BSN and an active RN license in Maryland, with at least two years of nursing experience.

The nurse will assess, plan, and deliver care in collaboration with the Interdisciplinary Group, document in facility charts, and educate families and staff on care techniques and equipment.

Qualifications

• Diploma, ADN and/or BSN from an accredited institution.
• Current Maryland State license as a Registered Nurse.
• Two years of nursing experience, or equivalent combination of education and experience.

Responsibilities

• Completes initial assessment of patient and family to ascertain home care needs.
• Prep & implement care plan with interdisciplinary group.
• Provide direct nursing care to patients in conjunction with the primary physician.
• Plan and supervise the delivery of care by the HHA.
• Coordinate with clinical manager, outside agencies, and community resources.
• Document in facility chart and maintain continuity of care.
