Job summary

Unity Hospice is seeking a Certified Nursing Assistant (CNA) to provide in-home and community-based care under the supervision of an RN. You will perform ADL tasks, mobility assistance, and vital sign checks while ensuring compassionate, quality care for patients in their homes and facilities.

The role involves cross-training for hospice residence support, with a flexible schedule including weekends, on-call and holiday shifts. A valid Wisconsin CNA license and reliable vehicle are required.

Qualifications

• Completion of certified nursing assistant program.
• Must be listed on the Wisconsin Nursing Assistant Registry.
• Valid driver’s license and access to a reliable vehicle.

Responsibilities

• Provide activities of daily living support.
• Deliver restorative and skin care.
• Assist with mobility and nutritional support.
• Monitor vital signs and report changes to RN.
• Provide companionship and patient education.
• Assist with medications, supplies, and documentation.
• Maintain competencies and communicate with the care team.
