Croí Health is seeking a Hospice Liaison to drive referrals and admissions across the South Shore in a full-time role. You will serve as the primary liaison to patients, physicians, hospitals, and care teams, ensuring timely referrals and high-quality care.

Responsibilities include developing relationships, expanding referral networks, coordinating with Intake and clinical teams, and representing Croí Health with professionalism to support sustainable census growth.

Formación

• 5+ years of proven sales or business development experience.
• Strong track record of generating new business and meeting sales goals.
• Excellent relationship-building, networking, and communication skills.
• Self-motivated, persistent, and comfortable with prospecting and cold calling.
• Healthcare or referral-based sales experience preferred.

Responsabilidades

• Develop and maintain strong relationships with physicians, hospitals, and referral sources.
• Identify and convert new referral sources; grow market share and census.
• Coordinate seamless referrals and admissions with intake and clinical teams.
• Educate referral partners and patients on services and value proposition.
• Conduct outreach activities to establish new relationships.
• Track referral activity and analyze performance to drive growth.
