Qualifications

• Registered Nurse licensed in the Commonwealth of Massachusetts and New Hampshire.
• BSN strongly preferred.
• 3–5 years of professional nursing experience; hospice experience preferred.

Responsibilities

• Provide skilled nursing care and prescribed treatments to patients in home/hospice settings.
• Coordinate patient care with interdisciplinary team and community resources.
• Attend Hospice Interdisciplinary Team meetings and contribute to care planning.
• Maintain documentation and use Hope Tool for assessments and symptom tracking.
• Participate in on-call rotation and weekend/holiday coverage as required.
