Job summary

Beth Israel Lahey Health seeks a Hospice RN Float to plan and deliver care to patients across service areas using the nursing process. Travel to any part of the BILH at Home service area will be required, and the base location may vary.

The role emphasizes coordinating with the interdisciplinary team, providing skilled nursing care, patient and family teaching, and ensuring high-quality end-of-life care. Certification in BLS is required, and nursing experience in hospice is preferred.

Qualifications

• Current RN license in Massachusetts and New Hampshire.
• 3–5 years of recent professional nursing experience.
• Minimum of one year hospice experience.
• BSN strongly preferred; current BLS.
• Valid driver’s license.

Responsibilities

• Manage patient care and coordinate services; communicate with providers.
• Provide skilled nursing care and prescribed treatments at patients' homes.
• Attend IDT meetings and participate in care planning.
• Instruct and supervise home health aides.
• Document care using electronic health records.
• Travel across BILH at Home service area as needed.
