Job summary

Qualifications

• Registered nurse with current licensure in MA and NH.
• Minimum 3–5 years nursing experience, home health preferred.
• Experience with OASIS data collection is preferred.

Responsibilities

• Coordinate patient care for a caseload with appropriate delegation.
• Complete initial assessments and develop care plans with patient/family.
• Administer medications and treatments per physician orders.
• Instruct and supervise home health aides on a 14-day basis.
• Participate in weekend/holiday/on-call rotations as needed.
