Job summary

HopeWest is seeking a part-time Hospice Float RN/LPN in Grand Junction, CO. The role provides flexible clinical support across hospice teams to ensure continuity of care for patients and families, with 12-hour shifts 7:00am–7:00pm and occasional weekend coverage.

The position requires an unrestricted CO RN license (or LPN), valid driver’s license, and at least 1 year of nursing experience; hospice or acute care experience is preferred.

Qualifications

• Unrestricted Colorado RN license required.
• Valid Colorado driver's license and auto insurance.
• 1 year of nursing experience required; hospice/home health experience preferred.

Responsibilities

• Provide flexible clinical support across hospice teams to ensure continuity of care.
• Deliver patient-centered nursing care, including assessments, care planning, and symptom management.
• Coordinate with interdisciplinary team and adapt to staffing needs.
