Job summary

HopeWest is seeking an RN/LPN to join our Hospice Float team in Grand Junction, CO. This part-time role (24 hours/week) provides flexible clinical support across hospice teams, ensuring continuity of care for patients and families.

You will perform assessments, plan care, manage symptoms and medications, and educate families while coordinating with the interdisciplinary team. The position requires an unrestricted Colorado RN license, a valid CO driver’s license, and at least 1 year of nursing

Qualifications

• Must have unrestricted RN license in Colorado
• Must have valid Colorado driver’s license, reliable transportation and auto insurance
• Compliance with training requirements for RN Float (PRN) Pool Program
• Minimum of 1 year of nursing experience
• 2 years in hospital, home health, oncology, critical care or hospice preferred

Responsibilities

• Provide flexible clinical support across hospice teams and service areas
• Deliver patient-centered nursing care throughout the hospice journey
• Perform patient assessments, care planning, symptom management and medication management
• Educate patients and families and coordinate with the interdisciplinary team
• Adapt to changing staffing and patient care needs and ensure compliance with policies
