Qualifications

• Must have unrestricted RN/LPN license in Colorado.
• Minimum of 1 year of nursing experience.
• 2 years in hospital, home health, oncology, critical care or hospice preferred.

Responsibilities

• Provide flexible clinical support across hospice teams and service areas.
• Perform patient assessments, create care plans, and manage symptoms and medications.
• Educate patients and families and coordinate with the interdisciplinary team.
• Adapt to staffing needs and ensure compliance with regulations and HopeWest policies.
