Job summary

Samaritan is seeking a dedicated Hospice Nurse to oversee the quality and expert care of assigned hospice patients in Camden/Atlantic/Gloucester counties, New Jersey. This full-time role emphasizes compassionate, patient-centered care within a not-for-profit home-based service.

Responsibilities include patient assessments, care planning, education, coordinating with the IDT, and arranging home visits, with a competitive salary and strong benefits package.

Qualifications

• Minimum of two years of nursing experience.
• NJ RN license required; BSN preferred.
• Bilingual is a plus.

Responsibilities

• Perform patient assessments for comfort and relief of symptoms.
• Establish and implement an individualized plan of care for each patient.
• Assess the need for patient and family education and provide education as needed.
• Obtain appropriate physician orders to achieve symptom management.
• Triage and arrange home visit schedule, demonstrating efficiency in time and mile management.
• Make referrals to additional IDT members and collaborate with social workers related to their involvement.
