Job summary

MJHS Hospice and Palliative Care in Brooklyn, NY invites you to join as a Hospice Field Case Manager. You will visit patients in community and facility settings to assess clinical and psychosocial needs, and document visits electronically.

You will develop and implement nursing care plans and advise on appropriate care options based on patient needs. The role requires NYS RN license, a nursing diploma or associate degree (BSN preferred), and a valid NYS driver’s license.

Qualifications

• Associates or Diploma in Nursing; BSN preferred.
• 2+ years in med/surg, community health, or oncology nursing.
• Current NYS RN license and NYS driver’s license.

Responsibilities

• Visit patients in community and facility settings to perform physical and psychosocial assessments.
• Develop and implement nursing care plans and recommend appropriate care.
• Document visits electronically and support on-call backup duties.
