Choice Health At Home is seeking a Hospice Field RN (Registered Nurse) for the Longview, TX hospice branch. The role involves comprehensive patient assessments, care planning, and education for patients and families while coordinating with interdisciplinary teams.

The ideal candidate has an active RN license, CPR certification, a valid driver's license, and relevant home health experience. Benefits include 401k, HSA, life and disability insurance, PTO, and potential fleet vehicle use.

Qualifications

• Current RN License is required.
• Valid Driver’s License is required.
• CPR Certification is required.
• Valid Auto Insurance is required.
• At least 2 years of Home Health RN experience preferred.
• HomeCare HomeBase experience preferred.

Responsibilities

• Conduct initial and ongoing comprehensive assessments and address physical, functional, psychosocial, and environmental needs.
• Determine admission and recertification criteria for appropriate care levels.
• Develop, document, and adjust the plan of care to ensure quality and continuity of service.
• Educate patients and families on self-care, nutrition, and dietary needs.
• Coordinate care, communicate changes to physicians, and document progress.
• Provide directions to Home Health Aide and LPN/LVN to maintain care quality.
• Document progress toward goals and share updates with care teams.
