Job summary

Choice Health At Home in Longview, TX is seeking a Home Health Field Registered Nurse (PRN) to deliver patient-centered care in patients' homes. Qualifications

• Current RN license.
• Valid driver’s license.
• CPR certification.
• Valid auto insurance.
• At least 2 years of Home Health RN experience preferred.

• Initial and ongoing comprehensive assessments and treatment on the patient’s physical, functional, psychosocial, and environmental needs as evidenced by documentation, clinical record, case conference, team report, evaluations, and ADLs.
