Job summary

Horizon Hospice of Colorado is seeking a Hospice Community Liaison to enhance hospice and palliative care referrals through community outreach in the Greater Denver Metro area. The ideal candidate will have a background in healthcare sales or community liaison roles, with strong communication and relationship-building skills.

This full-time, field-based position offers a competitive compensation package, including a base salary of $70,000–$82,000, plus performance incentives that can increase total annual compensation to around $100,000.

Qualifications

• Two or more years of healthcare sales, community liaison, or hospice experience preferred.
• Strong communication, presentation, and follow-up skills are essential.
• Must have a valid driver's license and reliable transportation.

Responsibilities

• Build relationships with healthcare partners and community organizations.
• Conduct outreach and educational presentations about hospice and palliative care.
• Coordinate timely referral handoffs to admissions teams.
• Track referral activity and outcomes using approved tools.
