Job summary

Your Health Organization is seeking a dedicated Continuous CNA to care for hospice patients in the Florence and Sumter areas. The role involves home visits, collaboration with the hospice team, and 7 on/7 off 12-hour shifts.

Strong patient care and communication skills are essential for delivering dignified, comfortable care. Responsibilities include ADLs, medication delivery, telehealth coordination, and documentation.

Qualifications

• Certified Nursing Assistant with active license in good standing required.
• Phlebotomy experience preferred; willing to train if not experienced.
• Experience in public health or home health is preferred.
• Experience with DME delivery or logistics operations is preferred.

Responsibilities

• Provide compassionate personal care to hospice patients in their homes, including bathing, grooming, oral hygiene, dressing, and toileting.
• Assist patients with safe mobility, transferring, repositioning, and fall-prevention support.
• Monitor and document patient condition changes and safety concerns.
• Deliver medications to patients' homes and reconcile instructions against lists.
• Coordinate telehealth visits between patients and pharmacists, providers, and therapists.
• Perform routine venipuncture for laboratory testing and proper specimen handling.
