Job summary

Your Health is seeking a dedicated Continuous CNA to support hospice patients across Florence and Sumter. This full-time, salary-based role involves a heavy home-care workload with 7 on/7 off 12-hour shifts, requiring reliable transportation and travel between locations.

Qualifications

• Certified Nursing Assistant (CNA) in good standing.
• Phlebotomy experience preferred; willing to train otherwise.
• Experience with home health or public health is a plus.

Responsibilities

• Provide compassionate personal care to hospice patients at home.
• Assist with mobility, hygiene, and daily living activities.
• Monitor patient condition and report changes to the nurse.
• Deliver medications and reconcile instructions per care plan.
• Coordinate telehealth visits with pharmacists and providers.
• Maintain safety, dignity, and comfort in all tasks.
